relieve the symptoms of advanced disease in order to maintain an optimum
quality of life.

support family* members through the process of bereavement before
and after the death of their relative.

enable patients and their families to participate in decisions and
plans relating to their future care and treatment to a degree dictated
by, and acceptable to, them. In order to encourage patients and their
families to be active participants in determining the place and style
of future care, we aim to provide information and advice as required
to assist this process.

contribute to the provision of a comprehensive, flexible and responsive
package of care available to the patient and his/her informal carers
achieved through close collaboration and joint planning with other
professionals in the Directorate, hospital and Primary Care Teams
and Social Services.

facilitate improved palliative care in hospital and community settings
through the provision of advice and education.

contribute to a working environment for all staff within the Directorate
that is mutually supportive and respects the contribution each individual
makes to the service received by the patient and their family.
