The Hospice provides specialist, palliative care for individuals and their families facing the diagnosis of a life threatening disease. Based in Medway and Swale it aims to enhance the care provided for, and the quality of life experienced by, those involved. This is achieved through the delivery of direct care, provision of advice and support, and by acting as an educational resource regarding the speciality of palliative care.

The Wisdom Hospice is part of the Medway Primary Care Trust and with the support from the Friends of the Wisdom Hospice, provides palliative care in patients' homes, day hospice and in other hospitals in the area. Psychosocial care, bereavement care and educational activities are also provided.

Referral to the services provided at the Wisdom Hospice is made by a health care professional - usually GP or Community Nurse at home or Consultant or Specialist Nurse in hospital. All referrals for the care of a patient and family at home need to be discussed and agreed with the GP and should be made on the Kent Specialist Palliative Care Referral Form.
